Book Description Health Care Without Medicare is a new practice manual for community-based care management. It describes the author's model for assisting frail elders, the physically disabled and the chronically ill to acheive their most often stated goals--to preserve their independence and their life savings. Community LifeCare Planning is described as the strategic use of personal, family and community resources to prevent hospitalization and nursing home placement among those most at risk--those with chronic health problems. The book was written collaboratively by two medical doctors, an estate planning attorney, a financial/retirement planner, three medical social workers (one of whom is a long-term care financing specialist) and two RNs. About the Author For Twenty years, Joseph A. Jackson, LICSW, CCM has worked in community mental health, hospice and home health care. He is president of ElderCare Advisors, Inc., a care management company based in Western New England. Mr. Jackson conducts workshops throughout the country on health care reform and community-based care management. He is licensed in clinical social work, a Certified Case Manager, and a member of the Natl Assoc. of Social Workers and the Natl Assoc. of Professional Geriatric Care Managers.
